Summary:                    [patch] Missing UniqueId header in ParkedCallGiveUp
and ParkedCallTimeOut manager events
Description: 
Manager events related to channels send contain the channel name and the
unique id. The ParkedCallGiveUp and ParkedCallTimeOut events are different
in that they do not contain the UniqueId.
The attached patch resolves this by adding the UniqueId header.
