Summary: chan_skinny doesn't respect callwaiting=no
Description:
In skinny.conf callwaiting=no has no effect for 7921G phones.
If A & B call C. Two lines ring on C's phone, even with only one line
defined, and callwaiting=no in skinny.conf
This is in 1.6.0-beta9 and 1.4.19

callwaitingv1 - skinny now respects callwaiting=no and skinny.conf. Has
extra stuff for debugging. Works on the old phones, but the newer ones work
differently in chan_skinny, so testing needed on new phones.
