Summary:                    [patch] set FAXMODE variable to let dialplan know

Description: 
Added FAXMODE variable which is set by SendFAX/ReceiveFAX to either "audio"
or "T38".

Also, reformatted application description a bit because it was distorted
by TAB characters.
