Summary:                    [branch] Report that channels are controlled by AGI

Branch: team/oej/fagi-adventures

This branch adds
- a flag in front of the application in "core show channels"
- a new manager header "AGIstate" for NewExten manager events to show that
this event was not generated by the dialplan, but by an AGI controller
- the same header to the show channels manager action responses

I was irritated that there was no way to see which channels where
controlled by AGI/FASTAGI/ASYNCAGI
