We are seeing the iax channel deadlock and cause asterisk to stop
processing calls intermittently. output of core show locks attached.

I did some digging through chan_iax2 but couldn't find anything that would
cause this issue.  However, I did update the "core show locks" output to
provide even more details that will hopefully provide a hint as to where
the problem is.  If you want to just apply the change to 1.4.13, you can
get the patch by running:
