Asterisk version: 1.4.11

Bug description:

Asterisk crashes (segfault) when an attended transfer to a queue is
performed and when EYEBEAM sofphone is used to make the transfer. This
crash can be easily reproduced as follows:

Ted Brown - we are seeing the same thing as you, no audio on calls into
queues, though it seems to be related to certain queue settings.
