Summary:                    Hook-Flash behaviour results in echo-cancellation
disabled on channel during call.
Description: 

With asterisk 1.4.11, during a standard call, 'zap show channel 1' reports
echo cancellation as 'ON'.  With call waiting, accepting the second call
keeps echo cancellation as expected.

When on a single call, pressing hook-flash to get a second dial tone turns
off echo cancellation.   If you successfully dial another number, echo
cancellation is renegotiated, and is set correctly for that call.   Hanging
up, will result in the call on hold 'calling back' the hand set.  When you
pick up the hand set, you hear a screech, and 'zap show channel 1' shows
echo cancellation as disabled, and you hear echo during the call after
resuming.
  
Upgrading to 1.4.12/13/14 changed this behaviour slightly.   Now echo
cancellation is disabled by hook-flash, and remains disabled, even when
hook-flash is used to resume the call on hold.

NOTE.

If you vary the above sequence.
1) call echo test
2) hook flash
3) listen for dial tone.
4) zap show channel 1 shows echo cancellation OFF.
5) press hook flash
6) resume call
7) zap show channel 1 shows echo cancellation ON.

This applies to 1.4.11 only.
Asterisk 1.4.12 and above show echo cancellation OFF at step 7
