No subject

voice mail is delviered to email. It's on for everyone who has a valid
email set.

If that is the case, the "Send Voicemail to Email only" should be
relocated to User Extensions. If the voicemail tickbox is selected, then an
smtp email address can be enterred. If not, the field can be
dimmed/disabled.

With this GUI setup you can cater for the 4 VM situations.

1. No VM on extension.
   Voicemail is unticked
   No smtp address set
   SMTP VM only unticked*

2. Traditional VM (no email)
   Voicemail is ticked
   No SMTP address
   SMTP VM only unticked*

3. Email only VM
   Voicemail ticked
   SMTP address enterred
   SMTP VM only ticked*

4. VM with email.
   Voicemail ticked
   SMTP address enterred
   SMTP VM only unticked*

Obviously an "SMTP address" is not enterred, then "SMTP VM only" cannot be
ticked.
