Summary:                    Asterisk crashes with error "Unable to masquerade"
Description: 
A masquerade which fails with "Unable to answer" results in a second
masquerade. The second masquerade fails with "Unable to masquerade". A
moment later, channel.c says "Unable to find channel in list" and asterisk
crashes.

Something was happening though to cause the masquerade to happen, like an
attended call transfer. What exactly was going on, right down to the order
of things...
