Description:
not sure what conditions caused it but it did happen and here is a quick
fix to stop it happening again

ok this happens in a EVENT_RELEASE_COMPLETE in chan_misdn the name has not
been set yet why ??
update_name is the function called when there is ...
EVENT_NEW_CHANNEL
the following only when there is a bchannel
EVENT_SETUP_ACKNOWLEDGE

EVENT_PROGRESS
