Summary:                    chan_local does not propogate cause codes
Description: 
When setting a cause code in a local channel context, e.g. Hangup(3), the
cause code is not passed along to be set in HANGUPCAUSE after the calling
Dial() completes.

I'd actually prefer to go a little closer to this route:  passing the cause
in the hangup control frame.  This should ensure that causes are passed all
the way along a chain.
