Summary:                    [patch] Failure reason missing in failed extension
Description: 
If you use the failed extension to trap any call failures in either a
spooled file or AMI originate you have no indication of why the call
failed.

This patch adds a channel variable called ${REASON} which holds the same
value as the reason: xx line in the originateresponse event
