Summary: [patch] native bridging support for chan_skinny
Description:
Possibly a bit rough, but so far it passes tests for
core and native bridging. P2P appears to need some
work, but I thought I might have enough to solicit
feedback.

Small cleanup effort to keep in step with Trunk. Tested to work with
7940 phone to SIP and OOH323 channels. 7920 and CIPC are having a
problem
with setting up the send channel, but that should not be hard to overcome.
