[Asterisk-bsd] random disconnects when checking voicemail

Case:
2 xp machines using diax, no firewalls.
1 4.10 fbsd, src and world current, asterisk-1.0.3_1 installed from ports
w/ WITHOUT_ZAPTEL= 1 added to Makefile, ipfw set to open and no special
entries in sysctl or rc.conf.
All running behind NAT on different ips; fbsd static, xp machines
dhcp'ing.
DMZ forwards to fbsd ip.

I use the following statement in extensions to check voicemail.

exten => _*123,1,VoicemailMain(s${CALLERIDNUM})
exten => _*123,2,Hangup

Caller Id is working properly and the voicemail account is the same as the
callerid number.

Problem:
About 80% of the time when I go to check voicemail, *123, (and it seems to
only happen when there IS either a new OR old vm) it will get to "you have
1"....then it is disconnected by the remote host.

This is what appears in the log file:
Jan 15 19:50:21 NOTICE[777]: I should never be called!
Jan 15 19:50:21 WARNING[777]: Wait failed (No such file or directory)

A google of the error provided nothing.

Also, I noticed that when it does get through and I try to change folders,
it will also get disconnected. Like its part of the same underlying
problem.

Any ideas?
