[Asterisk-bsd] Can't get FXO module to answer incoming calls.

I am using Asterisk 1.0.3 with FreeBSD Zaptel Drivers
and a TDM400P with one FXO module (TDM01B). I have
tried all different signalling types and have looked
over my config files very carefully. I can dial out
with no problem at all, but no incoming calls are
answered. Here are my files for reference:
