Looking for providers for DID origination with lotsof simultaneous

from just a few DIDs, and each with many ports (simultaneous calls) and an
overall high volume of minutes.<BR>(e.g., like a calling card business where
lots of users call a single number...)<BR><BR>Can anyone please provide some
guidance on providers and approaches to this? Most provider's
plans cater to individual phone plans where only 1 or 2 ports are
needed. I wonder if anyone has worked through this and found a
