Enswitch does this, and a huge amount more. It may be overkill though as 
it's a completed integrated package for commercial telephony services. 
Configurations start from USD 1100 per month, including software, 
installation, support (24/7/365 for emergencies), and upgrades. It 
scales to hundreds of thousands of users and tens of thousands of 
concurrent calls.

It's one of the few products that can handle conferences, queues, etc, 
across multiple machines in a cluster.
