Gavin,

Please see our Enswitch product, which uses Asterisk and OpenSIPS for 
telephony. It has full billing (prepaid and postpaid), invoicing, and 
payment. It supports packages of minutes per billing cycle with overage 
costs, with the minutes and destination sets configurable per rate plan. 
It includes a full set of hosted PBX features, SIP trunking, calling 
cards, and other commercial telephony business models. It's in 
production today from a few hundred users on a single machine to over 
150,000 users on large clusters.
