Our Enswitch product may be suitable. It's built around Asterisk and 
SER/OpenSER, and is in production today on systems of over 100000 users. 
It's aimed at hosted PBX providers and ITSPs (Internet Telephony Service 
Providers), but is also suitable for large CPE PBX systems such as what

This page discusses SER, but we also support OpenSER 1.2.1.

We have also enhanced many of the Asterisk features. In particular, we 
have made them work on redundant/failover clusters with many Asterisk 
machines delivering calls to users, which vanilla Asterisk does not
