The list which I started and never maintained was taken over by
astbill.com, I had some providers listed there although it was far from
complete and I found more info later on that I havent added to that
list ...  If they dont have the original info somewhere on
0xdecafbad.com is a list, although its most likely even more incomplete
as I dont have the time to track down every provider in every country :/

But its a start :)
