There a few out there. Check out http://www.vitelity.com
I believe that charge $1.49/DID for E911 and have a nice portal which 
allows you to update your E911 information on the fly.
Have had DIDs with them for a while now, and knock on wood... Have never 
had an issue.

(These statements are my experiences and opinion.
No facts can be derived from them.
You will just have to find out for yourself.)


And as always, US48 termination for $.01/min No Restrictions. SIP 
G729/G711. Quality!
Might even save you some money.
