Craig,

Enswitch can do this. The system owner makes multiple resellers, and set 
the prices for them in rate plans. The resellers then log in to the web 
interface, and create their own rate plans and customers. The customers 
then log into the web interface and buy DIDs, configure SIP accounts, 
etc etc. These customers can themselves be resellers, and there is no 
limit to the number of levels of resellers. Each reseller is responsible 
for billing their own customers, i.e. The system owner needs to know 
nothing about customers of customers.

All of "VoIP provisioning, plan creation, DID allocation, online CDRs 
and invoice generation." are supported.
