It is as easy as anything else done with asterisk.

I would be very, VERY skeptical of the viability of any operation
offering this rate.
The Modoc outfit, that sparked this discussion, doesn't even have a
proper web site set up. I certainly wouldn't bet my business on them
until the demonstrate a substantial track record of satisfied customers.

The bulk of the traffic on this forum would indicate that low-cost and
viable are mutually exclusive terms. 
Yes, many have built a termination/gateway service using asterisk. But
that's not what you were describing. You were describing "origination"
on your box, and handing it off to a VOIP terminator.
