[Asterisk-biz] FET Tax on phone service

There's no standard form, every carrier makes their own. VZ has one, 
Qwest has one. The only requirement is that it has to have all necessary 
information (i.e. the reason your customer claims the exemption).

Sprint's one is the only one I could find online.
