voipjet.com is $.013 for US48 and I can set the caller ID on the fly. I 
tested it with a 6 second call and sure enough it took $.0013 from my 
prepaid balance. Right now it says: Your current balance is $ 4.95180  
which means  I have 6.348461538 hours of time left. I am using it 
primarily to test a setup that calls my cell phone back(unlimited 
incoming minutes) and then allows me to make my calls. Calls where the 
caller ID should be the default are placed using lower-cost means. Calls 
where I want them to get one of my other DID or 800 numbers are placed 
using voipjet.com and I also use it when I need the occasional 
additional outgoing line. I have other land line and voip options I can 
fall back to when needed although the costs could run as high as $.04 in 
those situations.

You get 25 cents of free testing and after that you can test more with a 
prepay of as little as $5
