I use http://www.unlimitel.ca/ for my Toronto DIDs. They can deliver them
via SIP. I'd recommend them. I think they also have other Canadian cities.

http://www.livevoip.com/, http://www.xetricom.net/ and http://www.les.net/
also have Canadian DIDs.
