I would also be willing to host it and maybe build a minimal application
which lists out the DID providers and their Coverage areas.

How much interest is there in this?  I don't want to put it together and
then have two providers put their information in.
If anyone interested could you reply quickly just saying whether you
would use a site like this?

I think that if there is enough interest I would be willing to put
together a web app to allow providers to put this info on the web.  
I am frequently interested in this information myself.
